โœ… What Are Target Tasks?

Target tasks help you identify patients who may need further clinical attention because something is out of target. This proactive approach reduces clinical risk and supports better patient outcomes.

These targets are aligned with the national standards set in the QOF contract, so using them effectively can also help improve your QOF performance๐Ÿ’ก

 

๐Ÿ“Œ Where to find Target tasks?

In the Population View within the Proactive Monitoring Tool, you can filter for tasks and find the following clinical targets:

  • ๐Ÿฉบ Blood Pressure (BP) โ‰ค 140/80 or โ‰ค 150/90

  • ๐Ÿงช HbA1c โ‰ค 48 or โ‰ค 58 or โ‰ค 75

  • ๐Ÿซ€ LDL Cholesterol โ‰ค 2.0

 

๐Ÿ” Understanding Task Status

Target tasks use a different status system compared to other tasks in PMT:

๐ŸŸข Complete (Green):

โœ… Target achieved and completed this financial year ๐ŸŽ‰


๐ŸŸ  Opportunistic (Orange):

โš ๏ธ Last result was at target, but not yet done this financial year.
(Days overdue shows time remaining until year-end.)


๐Ÿ”ด Overdue (Red):

โ— Last result was off target.
(Days overdue shows how long ago the off-target result was recorded.)


๐Ÿ›  How to Use Target Tasks

The ๐Ÿ”ด Overdue group is the most important to focus on, as these patients are off target and likely need clinical intervention.

  1. Go to Population View.
  2. Use the Task Filter on the left-hand side.
  3. Select Target and choose the target you want to filter against.
  4. Apply Overdue Only to see patients whose last result was off target.
  5. Use Days Overdue to refine by time since last result.

    ๐Ÿ‘‰ Example: Find patients with HbA1c out of target for more than 3 months and invite them for a repeat test or medication review.

 

๐Ÿ“ฃ Recall Example

Identifying patients who have had their Blood Pressure checked this financial year but are out of target.

  1. Go to Advanced Search.
  2. Add criteria:
    • Target BP โ‰ค 140/80 and 150/90, Task Status = Overdue ๐Ÿ”ด
  3. Add another criterion:
    • Blood Pressure, Task Status = Complete ๐ŸŸข
  4. Add any exclusions as needed.
  5. Create a Recall for these patients and invite them for a review.
