The Analytics View offers selected users to view insights into achievement tracking and utilisation of the Proactive Monitoring Tool, and the rules which are enabled at your practice. We suggest using this feature when looking at general trends over time, helping you stay up to date with your monitoring and targets.

When navigating to the Conditions landing page, you will see a summary of the rules enabled for the selected practice(s), and the conditions covered by these rules.

In the menu bar, select the condition you'd like to explore further with a detailed dashboard.

Diabetes

Our Diabetes dashboard offers an overview of progress towards case-finding, monitoring, achieving targets and other proactive care requirements for your patients with diabetes. We also provide advice and recommendations on actions to improve in these domains and links to case studies and support material.

Along the left-hand side, there are progress trackers showing achievement of key indicators for DKCP, 3TT, QOF and other non-diabetes related care for patients with diabetes.

On the right-hand side, there are charts showing progress of key indicators over time. Click the dropdowns along the top of each chart to select the indicator you wish to explore.

Learning disability

Our Learning disability dashboard offers an overview of monitoring and other proactive care requirements for your patients with learning disability.LD

On the left-hand side, there are progress trackers showing achievement of monitoring for patients with learning disability.

In the middle section, there is a detailed view showing the percentage of Health Checks completed for patients with learning disability (based on IIF) and the numbers of patients who:

  • are up to date (for both their health check and action plan)

  • still need these completed

  • need their ethnicity recorded - clicking the red button will take you directly to Population View (in the desktop Proactive Monitoring Tool), to review these patients

The chart on the right-hand side shows the percentage of patients who are up to date with Health Checks over time.

There is also a summary of this information in table-format, which can be filtered by indicator using the dropdown above it.
