Automatically saving questionnaire responses to the patient record
Abtrace can automatically save selected patient questionnaire responses directly to the patient’s electronic health record (EHR).
When Autosave is enabled, eligible questionnaire responses are written directly to EMIS or SystmOne without requiring your team to manually review and accept each response.
Which questionnaires support Autosave?
Autosave is currently available for the following questionnaires:
- Lifestyle
- Smoking status
- Alcohol use (AUDIT-C)
- Physical activity
- Patient details
- Weight
- Height
- BMI
- Ethnicity
- Vaccination preferences
- Flu vaccine
- COVID vaccine
How to enable Autosave
- Log in to the User Management Portal.
- Select your organisation.
- Go to the Patient Portal section.
- Search for and select the questionnaire task you want to enable Autosave for.
- Select Gather information.
- Turn on Autosave for the questionnaire.

Once enabled, all subsequent patient responses submitted for that questionnaire type will automatically be written to EMIS or SystmOne.
What happens when a response is saved?
The patient's questionnaire responses and relevant clinical codes will be written to the consultation notes in the usual way.
Automatically saved responses will be labelled:
“Response recorded automatically”
Your practice will also receive an email confirming that the response has been successfully saved to the patient record.
What happens if Autosave is unsuccessful?
Autosave requires an active connection between Abtrace and EMIS or SystmOne.
If the response cannot be written to the patient record for any reason, it will not be lost. Instead, an email will be sent to your practice's nominated inbox so your team can review and manually accept the response in the usual way.
