How can I exclude patients who are monitored in Secondary Care?

"under secondary care monitoring" code can be used to excludes patients not monitored in Primary Care.

The required monitoring for medications are picked up by the Proactive Monitoring Tool based on specific rules. In some rules, as long as they have that specific medication issued on acute, then they will be included in the denominator. However, in others it might require the medication to be on repeat. This would determine the point at which the patient is picked up.
